Primary Health Care General Notes

In order to facilitate standardization of data at the Primary Healthcare (PHC) point of service (PoS), as well as interoperability and health system use, the Canadian Institute for Health Information (CIHI) in collaboration with jurisdictions and Canada Health Infoway Inc. (Infoway), led the development of the Draft Primary Health Care Electronic Medical Record Content Standard (PHC EMR CS). The Primary Health Care Electronic Medical Record Content Standard ( PHC EMR CS ) has since been updated and since renamed by CIHI to be the Primary Health Care Electronic Medical Record Minimum Data Set (PHC EMR MDS) for Performance Measurement version 1.0.  ITo support this MDS, Infoway continues to revise the PHC subsets where appropriate.
